To ensure the safety and understanding of all participants, everyone attending the Self-Defense Class must complete and submit a Release of Liability Form prior to participating in the session.

This form is a standard requirement and helps us maintain a safe and respectful environment for all.

✅ Please complete the form before the class begins.
📄 Forms will be available online and in person on the day of the event.
💡 Participants under 18 must have the form signed by a parent or legal guardian.
